Margaret Woods Allen Library Endowment
Prior to attendance in the program, a Physician Permission form must be completed and in the participants file. A form is provided for this requirement. This form must be updated yearly. In addition, each participant is required to have a TB Skin Test yearly.
If it is determined that the safety of the participants is in danger due to travel and/or exposure during inclement weather, the Senior Alive Adult Day Program will close for such period of time that danger to participants exists. Each participant scheduled to attend on that day will be contacted personally.
